Welcome to the 2008-2009 Patterson School Admissions webpage. We are privileged to have you and your child consider The Patterson School for your family.  We want to assure you that we will do everything we are able to ensure a positive and productive admission process.

Please complete the forms listed below and return to the school’s Admissions Office as a matter of urgency.

            1.      Application with $50 Application Fee
            2.      Transcript Release given to current school
            3.      Student Questionnaire
            4.      English Teacher Recommendation
            5.      Math Teacher Recommendation

        Once accepted, we will then need the following forms:

            1.      Physical Evaluation
            2.      Medical Emergency Form
            3.      Over 17 Permission to Communicate
            4.      Internet and Computer Use Policy
            5.      Immunization Record and copy of both sides of health insurance card
